This year's theme, "The Path Ahead," emphasizes the importance of effective leadership, team-based care and innovation as we navigate the monumental changes in primary care together.

Our goal is to foster an environment where multi-disciplinary teams can collaborate, gain valuable insights and leave with a renewed sense of optimism and inspiration. We’ll gather and learn about important initiatives underway that advance the Patient’s Medical Home in an Integrated Health Neighbourhood.

Event highlights:

• Engaging Sessions: Over the main two days, enjoy plenary and keynote speakers, a panel discussion with key primary care leaders, and interactive breakout sessions designed to inspire you to embrace the future and innovation.
