This presentation introduces how self-management programs support people living with chronic conditions and their caregivers.

It highlights the benefits of self-management for improving confidence, daily functioning, and emotional well-being. The session also provides an overview of the Better Choices, Better Health® program, emphasizing the role of peer support in its delivery. This will be followed by practicing a tool that helps us self-manage our chronic conditions. The presentation will touch on these key points:

  • How self-management programs help you
  • The Better Choices, Better Health® program and the role of peer support
  • Practice a tool for self-management
